Christmas Recovery

    1. Voucher must be activated before use. Activation is via this online form at www.physiowise.com.au or by presenting the voucher at a PhysioWISE clinic and providing the information required to complete the activation.
    2. The voucher entitles an existing PhysioWISE client to a discount of up to $20 off the normal price of a standard consultation. It can be used in conjunction with private health insurance, but if the gap is less than $20, the difference will be forfeited.
    3. It cannot be used in conjunction with any other offer or in conjunction with a treatment provided for DVA, CTP, Workers’ Compensation or other forms of 3rd party paid consultations.
    4. This voucher can be used at any time between activation and the 31st January 2013.
    5. Ony one voucher may be used per person.

It’s Your Birthday

    1. Voucher must be activated before use. Activation is via this online form at www.physiowise.com.au or by presenting the voucher at a PhysioWISE clinic and providing the information required to complete the activation.
    2. The voucher entitles an existing PhysioWISE client to a discount of up to $20 off the normal price of a standard consultation. It can be used in conjunction with private health insurance, but if the gap is less than $20, the difference will be forfeited.
    3. It cannot be used in conjunction with any other offer or in conjunction with a treatment provided for DVA, CTP, Workers’ Compensation or other forms of 3rd party paid consultations.
    4. This voucher can be used at any time between activation and the 31st December 2013.
    5. Ony one voucher may be used per person.

Winter “10+10”

    1. Voucher must be activated before use. Activation is via this online form at www.physiowise.com.au or by presenting the voucher at a PhysioWISE clinic and providing the information required to complete the activation.
    2. The voucher entitles an existing PhysioWISE client to a $10 discount off the normal price of a term of group classes.
    3. An additional voucher may be activated for a friend who is not currently a PhysioWISE client and will entitle the friend to a discount of $10 off a term of group classes also.
    4. It cannot be used in conjunction with any other offer or in conjunction with group classes provided for DVA, CTP, Workers’ Compensation or other forms of 3rd party paid consultations.
    5. Once activated, this voucher can be used for classes during Term 3 of 2013 (which commences 15/07/2013.
    6. Ony one voucher may be used per person.

Spring “10+10”

    1. Voucher must be activated before use. Activation is via this online form at www.physiowise.com.au or by presenting the voucher at a PhysioWISE clinic and providing the information required to complete the activation.
    2. The voucher entitles an existing PhysioWISE client to a discount of up to $10 off the normal price of a standard consultation when they bring a friend who is new to PhysioWISE.
    3. An additional voucher may be activated for the referred friend, who is not currently a PhysioWISE client, and will entitle the friend to a discount of up to $10 off the normal price of a standard consultation.
    4. The voucher can be used in conjunction with private health insurance, but if the gap is less than $10, the difference will be forfeited.
    5. It cannot be used in conjunction with any other offer or in conjunction with a treatment provided for DVA, CTP, Workers’ Compensation or other forms of 3rd party paid consultations.
    6. Once activated, this voucher can be used for a consulation between 1st September 2013 and 31st October 2013 (inclusive).
    7. Ony one voucher may be used per person.

Fitness First Members

    1. Voucher must be activated before use. Activation is via this online form at www.physiowise.com.au or by presenting the voucher at a PhysioWISE clinic and providing the information required to complete the activation.
    2. The voucher entitles an new PhysioWISE client to a discount of up to $20 off the normal price of an initial consultation. It can be used in conjunction with private health insurance, but if the gap is less than $20, the difference will be forfeited.
    3. It cannot be used in conjunction with any other offer or in conjunction with a treatment provided for DVA, CTP, Workers’ Compensation or other forms of 3rd party paid consultations.
    4. This voucher can be used at any time between activation and the 30th April 2013.
    5. Ony one voucher may be used per person.

Macquarie Centre Kid’s Club

    1. Voucher must be activated before use. Activation is via this online form at www.physiowise.com.au or by presenting the voucher at a PhysioWISE clinic and providing the information required to complete the activation.
    2. The voucher entitles a new PhysioWISE client who is a member of MC Kid’s club to a discount of up to $20 off the normal price of a standard consultation. It can be used in conjunction with private health insurance, but if the gap is less than $20, the difference will be forfeited.
    3. It cannot be used in conjunction with any other offer or in conjunction with a treatment provided for DVA, CTP, Workers’ Compensation or other forms of 3rd party paid consultations.
    4. This voucher can be used at any time between activation and the 31st June 2013.
    5. Ony one voucher may be used per person.

Referral from a GP or Health Professional

    1. Voucher must be activated before use. Activation is via this online form at www.physiowise.com.au or by presenting the voucher at a PhysioWISE clinic and providing the information required to complete the activation.
